Registration starts January 27, 2008.

Application Requirements Checklist

A) The following forms must be sent in with an application before admission is confirmed.

  1. Completed Application Form
  2. Birth Certificate
  3. Passport (if born outside U.S)
  4. Age Requirements:
    • Pre-k: age 3 by September 1,2008
    • KI: age 4 by September 1, 2008
    • KII: age 5 by September 1, 2008
    • Gr. 1: age 6 by September 1, 2008
  5. Baptismal Certificate (If your child has been Baptized.)
  6. Child's most recent Report Card (Grades KII through 8)
  7. Immunization Records
  8. New student interview & placement testing (if necessary) will be scheduled after the Application & Forms are submitted.

B) Your admission will be confirmed by an acceptance letter. Please include a check for $50.00 dollars (non-refundable deposit) made payable to the Pope John Paul II Catholic Academy when submitting your application. All applications can be mailed to the Pope John Paul II Catholic Academy office at 100 Savin Hill Avenue, Dorchester, MA 02125.
